The MRO sector is a real locus of abuse in many industries and self-regulation is non-existent. They are the “icing on the cupcake” of the drug-testing industry and make up many rules and principles on the fly. In a sense this group of physicians is charged with having both great expertise in clinical toxicology as well as a judicial temperament but in fact, the majority of them took a brief course in person or online. The prerequisite for the course was a medical degree and the price of tuition.
This “training pathway” has many similarities to the one in Addictionology that you have already described in so much detail. In occupational medicine, one has a ringside seat from which to observe the work of these physicians. Basically, when physicians are assigned a police function then a whole host of artifacts, distortions, and unintended consequences follow along in close succession.
